Dr. Kevin J. Smith, MD FRCPC performed an independent pain medicine assessment on Mr. David Temple. Dr. Smith is a licensed medical practitioner specializing in anesthesiology and chronic pain management. The assessment was conducted at the Aberdeen Hospital in New Glasgow, NS. Dr. Smith's role as an independent assessor was explained to Mr. Temple, and consent for release of the assessment report was obtained.
Motor Vehicle and Pedestrian Accidents Involving Mr. Temple
Mr. Temple was involved in multiple accidents that resulted in several injuries. Firstly, Mr. Temple was involved in a motor vehicle accident, which resulted in physical injuries such as multiple contusions and abrasions on his face, upper back, right thigh, right calf, left pretibial region, and left lateral ankle. This accident also resulted in more severe injuries including acute cervical strain, acute lumbar strain, a closed head injury, and a presumed occult fracture of the left cuboid. Secondly, on the same day, Mr. Temple was involved in another accident as a pedestrian resulting in injuries to his neck, upper back, lower back, and left ankle. The after-effects included complications such as pain, limited range of mobility, muscle spasms, tender areas, difficulty undertaking certain activities, insomnia, and weight loss. Post-accident, Mr. Temple experienced a range of complications including pain, limited range of mobility, muscle spasms, tender areas, difficulty undertaking certain activities, insomnia, and weight loss. He also endured psychological distress manifesting in symptoms related to post-traumatic stress disorder (PTSD). Despite seeking professional medical help, Mr. Temple's symptoms persisted and interfered with his daily activities and work.
1) Mr. Temple was involved in a motor vehicle accident and a pedestrian accident on the same day.
2) Suffered multiple physical injuries and psychological distress.
3) Despite professional medical help, his symptoms have persisted.
4) The sustained injuries have affected his daily activities and work performance.
5) The injuries sustained have not only inflicted physical damage but also have significantly impacted the mental health and daily life of Mr. Temple impacting his daily activities and employment capabilities.

Mr. Temple's Car Accident
Mr. Temple was involved in another motor vehicle accident that caused low back injuries and symptoms of post-traumatic stress disorder (PTSD).
He experienced ongoing anxiety, nightmares, avoidance behavior, and hypervigilance as a result of the accident. He had flare-ups of pain in his back, foot, and ankle and he has been receiving treatment from a psychologist and physician.

Diagnosis of Mr. Temple's PTSD
Mr. Temple was diagnosed with PTSD based on formal assessment and self-report measures.
He had trouble working in situations reminiscent of the accident, and his overall functionality and attitude towards work and recreational activities declined significantly. He was receiving cognitive-behavioral therapy, relaxation training, and hypnosis for treatment.
